Does this only apply to actions or what about action modifiers?

Action modifiers aren't successful or unsuccessful. They happen.
The only things that can be successful are actions and referendums. [and bleeds]

Also blocks are routinely successful (or not).

Damage can be inflicted "successfully" (e.g. Backstep, Target Blah, Rotschreck explaining its irrelevance).
