In the rulebook is stated that

"Some library cards have multiple Discipline symbols on the attribute bar. Some of the effects listed on these cards require one of the Disciplines listed, while other effects require another listed Discipline, and some effects require the vampire to possess several Disciplines. Each effect shows the icon(s) of the Discipline(s) required."

Name: Illegalism

Discipline: Celerity/Chimerstry/Presence
Requires an anarch. More than one discipline can be used to play this card. (D) Bleed.
[cel] Unlock this anarch if the bleed is successful.

Name: Smoke and Mirrors

Cardtype: Action Modifier
Discipline: Chimerstry/Obfuscate/Obtenebration
Requires an anarch.
[chi] +1 stealth with an optional maneuver in the resulting combat if this action is blocked.
[obf] +2 stealth. Not usable during a bleed or political action.
[obt] The blocking vampire gets -1 intercept.

Name: Anesthetic Touch
[BL:C1, LoB:C]
Cardtype: Combat
Discipline: Obeah/Auspex
Only usable at close range.
[aus] Strike: dodge.
[obe] Strike: hand strike. Combat ends immediately after the resolution of this strike.
[OBE] As [obe] above, with first strike.

Name: Alpha Glint
[BH:C]
Cardtype: Combat
Cost: 1 blood
Discipline: Animalism & Fortitude
Not usable in combat with an ally or an older vampire.
[ani][for] Strike: combat ends.
[ANI][FOR] Only usable before range is determined. Combat ends.

I have covered all possible combination of cards, including

a card who has three different (combined) effects
a card with three different and separate effects
a card with 3 different effects with 2 different disciplines
a card with 2 disciplines required to have effect.

In which case the discipline is required?

I think only if the discipline is used, so basically a True Brujah can play Illegalism with presence (ignoring it has celerity printed on it), and you can't use Hide the Mind to cancel a Anestethic Touch played with obe.

When a card is being played, it requires whatever discipline(s) you're using to play it at that time - even if there are other disciplines that could be used towards it. For example, Denial of Aphrodite's Favor could affect Illegalism if you chose to use it with Presence, but not if you didn't.

At other times (e.g. when in your hand, library), you can treat it as requiring all of the disciplines available. For example, you can fetch a Thaumaturgy/Visceratika card from your library with Spirit Summoning Chamber, even if you intend to play it only with Visceratika.
