This card is nothing spectacular. But just have a look:
Well Planned Attack

Do not replace until the end of the action
Enter combat with a ready tapped minion. Prevent one damage in the resulting combat.
As above but enter combat with any ready minion.
I saw him coming and *bam* hit him out of nowhere. Jimmy Dunn
This rush-action is just a completion of the different actions to let you enter combat. Most of them give maneuvers, some presses, some both. I'd find it nice to get one which lets you save blood in the combat.
I'd like the prevention to work even against environmental damage.
Would you use such a card? Only in multirush or even outside of it?

you only need Disarm to torporize with this in a 'non-combat-card'-combat. perhaps that's a little bit too easy?

Thematically, I'm not getting why a Fortitude action is a "Well Planned Attack". Anyone can plan an attack well, it doesn't require supernaturally tough skin. A discipline with some sort of scrying, prescience, or informational ability might do it (say, disciplines like Auspex or Animalism), but I'm not getting the Fortitude angle for the theme.
The flavour text also appears to be a very much opportunistic strike of opportunity.

I agree with jamesatzephyr on this one. Moreover, I'd say that fortitude is thematically a defensive discipline with a splash of suicidal intent (Dawn Op and similar), thus I hardly see rush action being inline or even needed.

+1 stealth action.
Enter combat with a ready tapped minion. Prevent one damage in the resulting combat.
As
above but prevent up to three damage from a single source.
As
above but enter combat with any ready minion.
Auspex makes more sense with advance planning. Since it requires two disciplines, the inital post's drawback (do not replace) becomes a +1 stealth bonus. (It shouldn't be worse than Nose of the Hound.)
